You have spent your career asking nothing. Ask for this.
This takes about ten minutes. There is no cost, no obligation, and no credit check. If you would rather talk to a person first, that is completely fine. Send us a note instead.
What we collect and who sees it
We ask for your contact details, your address, your agency and role, your household size, whether anyone at home depends on powered medical equipment, and optionally an income band. We ask for these because they are what the published eligibility criteria are actually decided on, and we do not ask for anything a decision does not require. We never ask for a diagnosis, a device list, a provider name, or an exact income figure.
Applications are readable only by the selection committee. They are never merged into our donor list and never enter a fundraising sequence. A suppression tag is applied automatically when you submit, so this is enforced by the system rather than by anyone remembering. We contact your supervisor only to confirm active service and call-back status, and we tell them nothing else about your application.
We keep non-selected applications for 24 months after the decision and then delete them. Records for installed systems are kept for the life of the equipment plus the audit period. You can ask us to delete your application at any time by emailing apply@lifelinepowerfoundation.org, and we will confirm in writing when it is done.
